COST IS $20.00-PLEASE MAKE CHECKS PAYABLE TO NORDHOFF
ATHLETICS.  ALL MONEY IS DONATED BY THE DOCTORS BACK TO
SUPPORT NORDHOFF SPORTS.

You can pick up a form in the office from the Athletic Secretary,
Roxanne Farrar. 

(This is the LAST PHYSICAL we will have this year.  Getting a
physical done at this time will make you eligible for the entire school
