Uniform Complaint Procedure
Any individual, public agency, or organization may file a written complaint of alleged noncompliance with the district by completing the appropriate notification form. The Uniform Complaint Form may be obtained from Kathy Smith, Executive Assistant to the Superintendent, at the district office at 414 E. Ojai Avenue, Ojai, CA 93023.
