California Healthy Kids Survey 2009-2010
Students in grades 5, 7, 9, and 11 take the California Healthy Kids Survey in the Spring. This is a mandated statewide survey that is administered every two years. The survey results will provide information about such topics as: school violence, after school safety, physical activity, nutrition, alcohol, tobacco and other drug use, and the achievement gap. Students taking the survey will have the choice not to answer any or all of the questions, or stop taking the survey at any time.
Survey answers are completely anonymous. Students in the 5th grade must have permission from their parent/guardian to participate in the survey. Students in grades 7, 9, and 11 take the survey unless their parents excuse them from participation by completing the appropriate form, which they may obtain from their school office. If you have any questions regarding this state mandated survey, contact John LeSuer at jleseur@ojai.k12.ca.us or at 640-4300 x1550.
